Payment for co-pays, deductibles, co-insurance, and all non-covered services is required at the time of the visit.

For patients with out-of-network insurance:
We welcome patients from all insurance plans, even if we are not in network with your provider. Our team will gladly prepare the detailed documentation you need to submit an insurance claim for potential reimbursement. Any benefits issued by your insurance will be paid directly to you.

If our physicians are considered out-of-network providers, payment is required at the time of service. Coverage and reimbursement will vary depending on your individual plan.
