Do You Really Need Strict Aftercare Following your Treatment?

For years, patients receiving botulinum toxin injections have been given long lists of post-treatment instructions:

"Don’t lie down for four hours"

"Avoid exercise"

"Stay upright"


Much of this guidance has been based more on theory and tradition than strong clinical evidence. A large multicenter study is now challenging whether extended post-treatment restrictions are truly necessary.


The Study

Researchers analyzed outcomes from 5,014 patients treated with botulinum toxin for upper facial wrinkles across six medical centers. Instead of giving prolonged aftercare instructions, patients were asked to follow only a 10-minute precaution protocol immediately after treatment.


The researchers evaluated:

  • Adverse effects (especially upper eyelid drooping, or ptosis)

  • Overall complication rates

  • Patient satisfaction at two-week follow-up


The Study's Results

The results were reassuring:

  • No cases of upper eyelid ptosis were observed.

  • Among 4,000 patients who returned for follow-up, only 5.99% experienced minor adverse effects — a rate notably lower than what is commonly reported in the literature.

  • Of the 2,010 patients who completed satisfaction surveys, 90% reported being very satisfied.


These findings suggest that extended behavioral restrictions after injections may not significantly impact safety or satisfaction.

Why Might Extended Restrictions Be Unnecessary?

Botulinum toxin works by binding quickly and locally at the neuromuscular junction. Once internalized into the targeted muscle, it is unlikely to migrate significantly.




What This Means for Our Patients

This means less disruption to daily routines, greater convenience, and reduced anxiety about “doing something wrong” after treatment.


Study Limitations

The study was retrospective, without a control group, and limited to short-term follow-up (two weeks). While the results are promising, stronger prospective and randomized studies are needed before fully redefining standard aftercare protocols. Given that, the conclusions do suggest that traditional guidance may not be rooted in science.
